- Filling out a complaint form with your personal details and details of your workplace complaint.
- Giving us as many supporting documents as you can.

- Lodging your workplace complaint by post or in person.
Lodge by post
Send copies of supporting documents, not originals. Write your name and date of birth on any copies of supporting documents you include.
Post your completed form, together with the copies of your supporting documents to:
